Sign-on Bonus: Full Time - $2000.00. Paid at 90 days of employment. Must have worked an average of 30 hours a week in the prior 90 days Part Time - $1000.00. Paid at 90 days of employment. must have worked an average of 20 hours a week in the prior 90 days Benefits: Medical, Dental, and Vision Insurance Paid Time Off (PTO) Accrual Life Insurance Short Term/Long Term Disability Coverage 401K Retirement Plan (for employees working 30+ hours per week) Job Description: We are currently seeking dedicated and compassionate LPNs to join our team. The LPN will be responsible for delivering high-quality healthcare services to our clients in their homes. The ideal candidate will have previous home health experience, strong documentation skills, and the ability to work independently. Responsibilities: Provide direct patient care in accordance with individualized care plans Conduct comprehensive assessments and document patient progress accurately Administer medications and treatments as prescribed by healthcare providers Communicate effectively with patients, families, and other healthcare professionals Ensure compliance with all regulatory requirements and company policies Utilize electronic visit verification (EVV) system for accurate clocking in/out Maintain confidentiality of patient information at all times Qualifications: Valid LPN license in the state of Colorado Previous experience in home health care is a plus Ability to work independently and prioritize tasks effectively Excellent documentation skills with proficiency in computer usage Strong communication skills and ability to collaborate with interdisciplinary team members Pay: $40 Per visit rate (Pay rate is PER VISIT) Job Types: Full-time, Part-time Application Process: If you are passionate about providing exceptional care and are interested in joining the Amazing Care team, please apply directly to this ad.
