Virginia Health Services is hiring Hospice Staff RN's, for both the Peninsula and Southside areas including : Kilmarnock Gloucester Williamsburg/York County Newport News/Hampton Virginia Beach Norfolk/Portsmouth/Chesapeake Suffolk/Isle of Wight County The Hospice Staff RN has the following responsibilities: Regularly assessing and reassessing the nursing needs of the Hospice patient. Providing Hospice nursing services, treatments and preventive procedures. Providing dietary counseling. Initiating nursing procedures appropriate for the patient's Hospice care and safety. Observing signs and symptoms and reporting to the physician and IDG members any unexpected changes in the patient's physical or emotional condition. Teaching, supervising and counseling the Hospice patient and family members about providing care for the patient. Supervising and training other nursing service personnel. Developing and re-evaluating the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care. Performing specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders. Attending team conferences. Maintaining records as required by Hospice. Following the policies and procedures of Hospice. Observing confidentiality and safeguards all patient-related information in compliance with HIPAA regulations. Always communicating to the supervisor if unable to meet a patient's need or perform a procedure. Participating in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties. Coordinating the implementation of the plan of care for patients residing in SNF, NF, ICF or MR. Complying with agency infection control policies and protocols. Assisting with orientation, teaching and training as requested. Requirements: Graduate of an accredited School of Nursing and holds current Virginia RN license. Minimum of 2 years experience as a registered nurse, preferred. Active patient contact within past three years, preferred. Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Why join Virginia Health Services?
