Virginia Health Services is hiring Physical Therapist's for Virginia Health Home Care for both the Peninsula and Southside areas including : Kilmarnock Gloucester Williamsburg/York County Newport News/Hampton Virginia Beach Norfolk/Portsmouth/Chesapeake Suffolk/Isle of Wight County The Physical Therapist has the following responsibilities: Screening of all patients. Evaluating and treating all patients who could benefit from intervention as referred by a state approved referral source. Appropriately documenting progress toward identified objectives. Communicating patient status and needs to the patient, the patient's family, the staff and other professionals. Establishing and maintaining courteous and helpful interaction patterns with staff, patients and families which reflects sensitivity to individual needs and a problem-solving approach. Actively integrating therapist's role into facility team. Directing the activities of physical therapy assistants in a manner which reflects an understanding of the functions of each position and facilitates their development within those boundaries. Educating staff, other health professionals, and the general community about physical therapy and VHRA services on a consistent basis. Attending facility patient care, utilization review, marketing administrative staff/department head, and family conference committee meetings on a consistent basis. Establishing and maintaining a good working relationship with other health care professionals and colleagues in the community. Making appropriate referrals to other members of the rehabilitative team (OT and ST.) Developing and implementing new programs/techniques to enhance physical therapy services and facility utilization. Adhering to established facility policies and procedures related to environmental safety, orderliness, risk management, infection control and emergency situations. Adhering to the requirements of the patient's Bill of Rights. Requirements: Must be a graduate of a APTA accredited school of physical therapy. Must hold a valid state license (where appropriate.) Must show evidence of a current satisfactory physical examination and/or evidence of freedom from communicable diseases, where required by the state. Must be capable of easily lifting 50 pounds of dead weight alone. Must have the ability to walk, bend, stand and reach constantly during at least an 8 hours day/shift.
